Off-label prescribing is legal and common in medicine, and longevity clinics have built a program type around it. The candidates are approved drugs with a mechanistic argument for slowing aging: rapamycin through mTOR inhibition, metformin through metabolic signaling, GLP-1 agonists through weight and inflammation. The human evidence is thin but real. The PEARL trial, published in 2025, randomized 115 healthy adults to 5 or 10 milligrams of compounded rapamycin weekly or placebo for one year. Low-dose rapamycin was generally well tolerated, and women on the higher dose showed modest gains in lean mass and self-reported pain. The trial did not measure lifespan, disease incidence, or biological age in a way that confirms a longevity benefit, and the authors said so. The proposed metformin trial in aging, TAME, has not completed. GLP-1 drugs have strong outcome data, but in people with obesity or diabetes, not as a longevity treatment for people at normal weight.
This is the program type where a careful clinic and a careless one look most alike from the outside. Both hand a healthy 50-year-old a prescription. The careful one measures first, doses conservatively, monitors immune and metabolic markers, has a stop rule, and coordinates with the patient's other physicians. The careless one runs a questionnaire and a subscription. The difference is invisible on the website and obvious in the follow-up.
